Jennifer Talley, MSN, FNP-C

FAMILY NURSE PRACTITIONER

Originally from Michigan, Jennifer has proudly called the Charleston area home for the past 10 years. She earned her Bachelor of Science in Nursing from Winston-Salem State University and her Master of Science in Nursing from the University of North Carolina at Charlotte, where she completed her Family Nurse Practitioner education. As a Board-Certified Family Nurse Practitioner, Jennifer is dedicated to helping patients achieve optimal health through personalized, evidence-based care.

Jennifer’s diverse clinical background includes experience in emergency care, pain management, functional/integrative/ regenerative medicine, providing her with a comprehensive perspective on patient care across a variety of healthcare settings. She has also pursued advanced training in integrative/functional medicine, hormone optimization, metabolic health, medical weight management, longevity medicine, and peptide therapies. Jennifer believes the best healthcare begins with listening, educating, and partnering with patients to identify and address the root causes of illness rather than simply treating symptoms.

Jennifer’s passion for integrative medicine is deeply personal. After years of experiencing chronic pain/ inflammation, metabolic health challenges, and unexplained symptoms herself, she understands the frustration of searching for answers. Through a root-cause, evidence-based approach, she experienced firsthand the profound impact of addressing the underlying causes of illness, not simply managing symptoms. That journey inspired her mission to help others restore vitality, optimize their health, and discover that lasting wellness is possible.

Beyond caring for patients in the clinic, Jennifer is passionate about educating her community. She enjoys hosting wellness events and educational seminars that empower individuals to better understand their health and make informed decisions about prevention, longevity, and whole-person wellness.

Outside of the clinic, Jennifer enjoys spending time with her husband and two children and her church family. You’ll find them all either on the ice figure skating and playing hockey or finding adventure outdoors. She considers it a privilege to partner with her patients on their wellness journey and is committed to helping them live healthier, more vibrant lives.

Clinic Philosophy

At Perform Medical, we believe wellness should feel transformative, empowering, and refined. That’s why our philosophy is built on three pillars:

A doctor in a white coat is reassuring a couple during a consultation in a bright medical office. The doctor is smiling and gently placing a hand on the woman’s shoulder while the couple looks calm and appreciative.

Wellness-First Medicine

We focus on identifying and treating root causes—not just managing symptoms. Our integrative approach to health combines modern science with natural, restorative therapies. Whether it’s hormone therapy, sexual rejuvenation, or weight loss, our care is always tailored, targeted, and results-driven.

Rejuvenation and Vitality

Aging doesn’t have to mean decline. Our treatments are designed to restore your body’s natural vitality—enhancing energy, libido, strength, and appearance. We help you feel younger, stronger, and more in control of your body at every stage of life.
